All Cargo tendered for transport is subject to inspection. I hereby give the air Carrier consent to inspect this Cargo. Sky Cargo USA is not responsible for any damages, Loss or delay caused by the Airline or third Party. Any Goods unclaimed after 5 days of notification of arrival will incur 20% storage fees for each day thereafter. Any Goods left after 60 days will be disposed off.
PLEASE YOU/YOUR RECIPIENT SHOULD INSPECT THE GOODS PROPERLY BEFORE ACCEPTING.

Please complete and submit this form before coming to our office to ship something. This will save you and us paper work time.

When an air freight shipment is picked up by a driver or dropped off at our facility we are required to verify and record the identification number of the shipper. Please complete this form for every air freight shipment.

A valid form of ID is required from each individual who tenders cargo for transportation at time of acceptance. An expired ID is not valid.

1) Photo ID issued by a government authority -OR-
2) Two other forms of ID at least one of which must be issued by a government authority.

Record the information below, No blanks are allowed "N/A" (not applicable) must be used to indicate that information was reviewed and does not apply.

  • All Cargo Tendered for Transportation Is Subject To Inspection
  • Type of ID Required:  Driver License, USA Passport, State ID, Military ID, US Green Card. One of these is Okay
  • Sky Cargo USA, LLC does not Carry Prohibited items, Alcohol, Cologne, Perfume or any Items with Alcohol or Chemical in Them.
  • All payments are due when Items are Tendered.
  • No Cash on Delivery (COD). Payment in Foreign  Countries is subject to Management Approval.
  • Sky Cargo USA, LLC. is not responsible for any Demages, Loss or Delay Caused by the Airline or third Parties, but we will assist in ratifying the issue.
  • All Items continuing From the First Port of Landing is Subject to Insurance Coverage/Transportation Fees.
  • Sky Cargo USA, LLC. Responsibilities are to our Customers Tendering Items and NOT to Other Party/ies.
  • Items Unclaimed After 5 Days Of Notification of Arrival will INCUR 20% Storage Fees Fo Each Day. Thereafter, any item left unclaimed after 60 Days will be Disposed of.
